What is it and when will employees receive it?


As part of the Affordable Care Act (ACA), commonly known as health care reform, the federal government requires employers to show whether they are offering affordable health insurance to eligible employees. In order to do that, Human Resources will be sending eligible employees a 1095-C form no later than March 2, 2017.

The 1095-C form states whether you were offered qualified, affordable health insurance for the whole year in 2016, or for certain months, whether or not you  were enrolled in Cigna or Kaiser during 2016. Form 1095-B – is provided by  Kaiser if you were enrolled under our group plan during 2016.

These forms are required as supporting documents when completing your 2016 tax return. You are not required to submit the form with your tax return.
