Used for Colorado residents out-of-state due to attending school, military service, working out of state, seasonal work, temporary work or a disabled vehicle.

Application for Persons with Disabilities Parking Privileges
Used to obtain handicap plates and/or placards; must be signed by a physician licensed to practice in Colorado or a Commissioned Medical Officer.

Motor Vehicle Bill of Sale
Used for private party motor vehicle sales.

Power of Attorney for Motor Vehicle Only
Used to give another individual the authority to sign for someone else.  This form is limited to motor vehicle transactions.

Verification of Vehicle Identification Number
Used to verify vehicle identification number, physically found on the vehicle with the paperwork being submitted.
