Birth and Death Certificates (Vital Statistics)


Birth Certificates

Individuals who were born in Colorado (regardless of County of birth or residence) may purchase state-certified copies of their birth certificates at the Central Records Office, which is located in the City and County of Broomfield Building at One DesCombes Drive.  The hours of operation are 8 a.m. to 5 p.m. from Monday through Friday.  Click here to obtain driving directions to One DesCombes Drive.

Broomfield can issue state-certified birth certificates for any individual born in Colorado since 1903. Certified birth certificates are needed to obtain a Colorado driver's license, identification card or driver's permit, as well as for some employment purposes, U.S. passport applications, school enrollment, extracurricular activities, sports, and to apply for public assistance programs.  Hospital birth certificates with baby's footprints are not considered legal documents.

Identification Required

In order to receive a copy of a birth certificate, the individual making the request must have a valid state ID or driver's license and must prove their relationship to the person on the certificate. In lieu of a photo ID, the applicant may submit two of the following forms of identification: 

  • Current paycheck stub that includes the company name and applicant's name;

  • Social security card that is not laminated;

  • Current car registration or car title. 

If the applicant cannot provide acceptable documentation, a parent, spouse, grandparent, adult sibling or adult child may provide appropriate identification to request the birth certificate as long as proof of relationship, such as a state-issued birth certificate or marriage license, is presented.

Fees

The cost for birth certificates is $17.75 for one copy and $10 for each additional copy of the same record.

Death Certificates

Death certificates for individuals who passed away in Broomfield County after November 15, 2001 may be obtained at the Health and Human Services Department at 6 Garden Center in Broomfield.  Hours of operation are 9 a.m. to 4 p.m., and the last application is accepted at 3:45 p.m.  Click here to obtain driving directions to this location.

Certified copies of death certificates may be issued to:

  • Parents,

  • Grandparents*,

  • Stepparents*,

  • Siblings*,

  • Spouse*,

  • Adult children/grandchildren*, or

  • Legal court-appointed guardian*.

    *Proof of relationship is required.

Colorado Revised Statutes require that all applicants have a direct and tangible interest in the vital records being requested.  Proof of relationship such as a state-issued birth certificate or state-issued marriage license is required.
