Birth and Death Certificates (Vital Statistics)

Birth Certificates

Vital Statistics is responsible for collecting data and issuing birth and death certificates.  The collection of health data regarding causes of death, low birth weights, adolescent pregnancy, and other health trends provides information about factors, which drive the need for Public Health programs.  Public Health is able to issue copies of birth certificates for anyone born in Colorado, regardless of the county in which they were born. The Health and Human Services Department (HHS) issues state certified birth certificates for any individual born in Colorado since 1903. Certified birth certificates are needed to obtain a Colorado Driver's License, identification Card or Driver's Permit, as well as for proof of employment, U.S. Passport applications, school enrollment, extracurricular activities, sports, and to apply for county social services and programs.  Hospital birth certificates with baby's footprints are not legal documents.

Certified copies may be issued to:

  • The registrant (person named on the birth certificate),

  • Parents,

  • Grandparents**,

  • Stepparents**,

  • Siblings**,

  • Spouse**,

  • Adult children/grandchildren of the registrant**, or

  • Legal court appointed guardian**.

    **Proof of relationship is required.

 Identification Required

In order to receive a copy of a birth or death certificate, the individual making the request must have a valid state ID or drivers license and prove their relationship to the person on the certificate.  (This requirement does not include parents.)  In lieu of a photo ID, the applicant can submit two of the following forms of identification: a current paycheck stub with the company name and applicants name; a social security card that is not laminated; a current car registration, car title or voter registration.  If you cannot provide acceptable documentation, it is suggested that you ask a parent, spouse, grandparent, adult sibling or adult child who can provide appropriate identification to request the birth certificate.  Proof of relationship is required such as state-issued birth certificate or state-issued marriage licenses.

 Hours and Fees

The address for the Broomfield Vital Statistics Office is 6 Garden Center (NE corner of Midway and Hwy 287).  The office is open from 9 am to 4 pm , Monday thru Friday. Applications for same day processing will not be accepted after 3:45 p.m. The cost for birth certificates is $17 for one copy and $10 for each additional copy of the same record.  Effective July 1, 2008, the cost for the first birth certificate will increase to $17.75 with the cost for additional copies remaining at $10.  Birth certificates are printed in the office.  The wait time in Broomfield is about 15 minutes.  For additional information please call 720-887-2270.

 Birth certificates can be ordered through the mail and must include a legible copy of the applicant's state id or drivers license and an application.

Death Certificates

HHS also provides certified death certificates for individuals who passed away in Broomfield County after November 15, 2001.  The cost for a death certificate is $17 for the first copy and $10 for each additional copy.

Colorado Revised Statutes require that all applicants have a direct and tangible interest in the vital records being requested.  Proof of relationship such as a state-issued birth certificates and state-issued marriage license is required.
