Family

The HOPE nurse home visitation program offers eligible women and families the opportunity to receive home visits from a public health nurse to promote healthy pregnancies, healthy children, and healthy families. The HOPE program is offered to participants FREE OF CHARGE. There are two ways that families can participate in the HOPE program.

PRENATAL HOPE   FAMILY HOPE
  • FREE nurse home visitation program for pregnant women who live in Broomfield County. 
  • Women who are eligible will receive home visits from a public health nurse every 1-2 weeks during their pregnancy and up to their child’s 2nd birthday.
  • Clients may be first time moms or may already have children.
  • Home visits begin as early as possible in the pregnancy and continue up to the child’s 2nd birthday.

  •  FREE nurse home visitation program available to eligible Broomfield families who are looking for a little extra support.
  • Visits occur every 1-2 weeks for a period of no more than 16 weeks (4 months). You and your nurse will decide the ideal length of time for you.
  • Home visits provide education, assistance in goal setting, and referrals to community resources. Visits are tailored to a client’s needs and may occur once or may continue every 1-2 weeks up to 16 visits.

Your HOPE Nurse Can:
  • Help you and your children have the best possible health (nutrition, healthy behaviors in pregnancy, behavioral health, children’s health, etc.)
  • Help you set goals for your personal and family future and explore how you can reach these goals (education, employment, family planning, self-sufficiency, etc.)
  • Help you connect to family and community resources (support systems, housing, employment, local resources, etc.)
  • Help you build a positive parenting relationship with your children (child development, parenting ideas, tips and techniques that can help you become a more effective parent)
  • Help you make your home a safe place for your children to live and play (child-proofing, fire safety, first aid, CPR, etc.)
