HOPE Nurse Home Visitation Program

Broomfield HHS has recently launched a new nurse home visitation program for clients who are eligible for the Temporary Assistance to Needy Families (TANF) program.  The home visitation program, called HOPE (Helping Our Families by Promoting Empowerment) offers eligible clients and families the opportunity to receive multiple home visits by a Public Health nurse to promote healthy pregnancies, healthy children, and healthy families.  The program has two components, a Prenatal program and a Family program.

The Prenatal HOPE program is designed for pregnant women with medical, social or behavioral risk factors.  Clients may be first time moms or may already have children.  All first time moms are offered Broomfield’s Nurse Family Partnership (NFP) program administered through Jefferson County.  However, if the NFP program is unable to accept the referral, the client will be offered the HOPE program.  Home visits begin during the pregnancy and continue through the child’s second birthday.  Some examples of risk factors include previous low birth weight infants, tobacco or substance use, poor maternal weight gain, low level of education, history of depression, lack of social support, history of child abuse, and high levels of perceived stress. 

Using evidence-based curriculum and theory-driven models, the nurse conducts comprehensive family assessments, facilitates goal setting that addresses critical risk factors and, building on the family’s strengths, partners with families to provide education and assistance on a variety of topics including pregnancy and childbirth, family planning, child development, parenting techniques, relationship building, social support networks, and locating community resources. 

Family HOPE is designed for any family seeking additional support or who would benefit from a short-term intensive program in an effort to reduce risk factors associated with child and family violence.  Home visits provide education, assistance in goal setting, and referrals to community resources.  Visits are tailored to a client’s needs and may occur once or may continue every 1 to 2 weeks up to 16 weeks.
