Child Health Plan Plus (CHP+)

The Child Health Plan Plus (CHP+), is a full service health plan for Colorado's uninsured children age 18 and under. CHP+ is a managed care health plan and is not Medicaid. Coverage is limited to a comprehensive package of benefits designed specifically for children and youth. This program offers low-cost health and dental insurance to families who make too much money to join Medicaid but not enough to afford private insurance.  Children in CHP+ get their health services in an HMO (Health Maintenance Organization).  An HMO is a group of doctors, nurses, hospitals and othe rproviders who work together to give children in CHP+ health care.  Members must visit a doctor within the HMO. Eligibility Requirements:

Children may qualify if:

  • They are 18 years old or younger.

  • They live in Colorado.

  • They don't qualify for Medicaid.

  • They don't have other health insurance.

  • Their families meet the CHP+ income limits.

  • They are U.S. citizens, legal permanent U.S. residents for at least 5 years, an asylee, or refugee.

Pregnant women may get health care right away while they wait for CHP+ to review their applications.  This is called presumptive eligibility. 

To apply:

Applications (in English or Spanish) may be obtained in the following ways:

1) Online by visiting the Children's Health Plan Plus website at www.CHPplus.org or the Colorado Public Health Insurance for Families (CPHF) website at www.chcpf.state.co.us

2) By visiting the Broomfield Health and Human Services Department at 6 Garden Center, Broomfield, CO 80020.

3) By calling the Broomfield Health and Human Services Department at (720) 887-2287 or CPHF at 1-800-359-1991.

The application must be completed and submitted to the Broomfield Health and Human Services Department.

Cost:

Depending on your family size and income, you may need to pay an enrollment fee once each year, and co-pays (what you pay each time your get a service). Some families pay nothing, while some families pay up to $35 per year and $2 or co-pays for some medical and dental services. Pregnant women have no co-pays.
