Form 1095-C for Reporting Health Coverage


THE NEW 1095-C TAX FORM -
What is it and when will employees receive it?

As part of the Affordable Care Act (ACA), commonly known as health care
reform, the federal government requires employers to show whether they are 
offering affordable health insurance to eligible employees.  In order to do that,
Human Resources will be sending eligible employees a 1095-C form no later
than March 31, 2016. 

The 1095-C form states whether you were offered qualified, affordable health insurance for the whole year in 2015, or for certain months, whether or not you
were enrolled in Cigna or Kaiser during 2015. Form 1095-B – is provided by
Kaiser if you were enrolled under our group plan during 2015.
 
These forms are not required for completing your 2015 tax return.  You are not required to submit the form with your tax return.  Similar to last year, taxpayers
can prepare and file their returns using other information about their health
insurance.
