Looking for a birth certificate, death certificate or information on vital statistics? Broomfield’s Department of Health and Human Services (HHS) can help.

For the past year, Broomfield’s Clerk and Recorder has been responsible for vital statistics records. Now this function is within the purview HHS. The transfer will allow HHS to collect and assess data important in identifying health and human services trends.

Residents may contact HHS at 720-887-2270 with questions.

Applications for specific certificates can be obtained by mail but must be submitted in person with appropriate documentation.

Certified birth and death certificates cost $15 for the first copy (or a search that may not result in an issuance) and $6 for each additional copy issued at the same time. There are also special processing fees ($20 or $7), depending on the type of change that’s made.

HHS is located at 6 Garden Center, north of Wells Fargo Bank.
