A Program for Children and Youth with Special Health Care Needs
Broomfield’s HCP Program provides information and resources for children and youth with special health care needs from birth to 21 years of age. The program is free to anyone needing help to address a child’s behavioral, developmental, medical, and/or social needs. Please call Broomfield’s Public Health Office at 720.887.2220 and a public health nurse will discuss more about what HCP has to offer.

HCP Services
Families turn to HCP for information, resources, and referrals to valuable community support in Broomfield. HCP promotes communication between families, providers and community resources by connecting children and youth to the care they need. Families may also gain a greater understanding of their child’s medical condition and coordinating their child’s health care. This can lead to a more successful and fulfilling experience at home, at school and in the community.
Referrals to Broomfield’s HCP Program can be made by anyone. If you know a child you think might benefit from the program, please call our office at 720.887.2220 or complete a referral and email to publichealth@broomfield.org or fax to 720.887.2229.
