Health First Colorado members can access their household’s medical card online anytime through their PEAK account: PEAKHealth App or online: coloradopeak.secure.force.com/.

Health First Colorado (Medicaid) is a public health insurance for low-income families, children, pregnant women, persons who are blind or disabled, and the elderly.
If you do not qualify for Health First Colorado, you can apply for CHP+ or Connect for Health Colorado.

Long Term Care includes nursing facility care, home and community-based services in which medical providers come into the home to render the necessary services to keep a disabled individual from being institutionalized in a nursing facility or a hospital.

- If you already receive Colorado Medicaid benefits, complete and submit the Request for Long Term Care form and submit to yourbenefits@broomfield.org.

Home and Community Based Services (HCBS Waivers):
A waiver is an extra set of benefits you could qualify for in certain cases. These benefits allow you to remain in your home and community. Waivers have extra program rules and some programs have waitlists. For additional information on waivers and programs click the links below.
Medicare Savings Program (MSP) assists individuals with limited income and resources pay for some or all of their Medicare deductibles and coinsurance. Colorado residents may apply for this program if they have Medicare.

Effective July 1, 2020, Broomfield County Department of Human Services will no longer provide the Healthy Communities Program for Health First Colorado (Medicaid) and Colorado Child Health Plan Plus (CHP+).

Child Health Plan Plus (CHP+) is public low-cost health insurance for certain children and pregnant women. CHP+ is for people who earn too much to qualify for Health First Colorado (Colorado's Medicaid Program), but not enough to pay for private health insurance.
CHP+ costs vary depending on your household size and income. CHP+ members have to pay an annual enrollment fee. Some CHP+ clients may also have to pay co-pays to their health care provider at the time of service.
