Submit a Complaint

Community members can report public health concerns to Broomfield Public Health and Environment. 

If you or someone in your household is/was sick (vomiting, diarrhea, stomach ache, etc.) and suspect it was caused by food, fill out the below form:

Community members can submit a public health complaint by creating an account on My Health Department and completing the form

Complaint Examples:

Broomfield Public Health and Environment takes complaints for:

  • Restaurants, grocery stores, and other retail food locations
  • Onsite wastewater treatment systems
  • Open burning
  • Recreational water such as public swimming pools
  • and more
