Collaborative Management Program (CMP)
The Collaborative Management Program (CMP) is here to make things easier for families. We bring schools, health care, child welfare and other community partners together to support you in one connected plan—so you don’t have to navigate services alone.Through strong partnerships, open communication and shared goals, we work together to achieve the most effective outcomes for our community.
Our goal is simple:
To help families get the right services at the right time. With CMP, your voice matters. We work with you by bringing families and support teams together to create real solutions that work.
This program serves Broomfield County residents with a child in the home from birth to 21 years.
